The online application window for Uttar Pradesh National Health Mission Community Health Officer (UP NHM CHO) post has been closed long back and now it’s time for the written examination to be conducted in a few day’s time on 16th June 2019. The UP NHM department has now released a notification on its official website, announcing the availability of the admit card or call letter for the examination. The link for viewing and downloading the call letter has been posted on the department’s official website “” which will be active till the date of the examination. A candidate must keep his Registered Mobile Number and his Date of Birth ready in hand when downloading the admit card because it will be required to log in to the website for downloading the call letter. Important Dates For The Issue Of Admit Card:

The UP NHM department has announced and published the link on its website for downloading the admit card for the CHO post’s examination. The online link for the admit card has been activated on 06th of June 2019 which will be available for 10 days 16/06/2019 i.e. till the date of examination. It is advised to download the call letter days before the examination to avoid any last minute technical glitch or website failure.

How To Download The, Admit Card:

The steps to download the admit card is simple and easy to follow, which are explained below-

Step 1– On your web browser type the link “” which is the official website of the UP NHM department, which will take you to the homepage of the website.

Step 2– Once you land on the homepage, search for a hyperlink with the words ‘Admit card for the online CHO post examination 2019’ and click on that hyperlink once you find it.

Step 3– Once you click on the hyperlink, you will land on an E-Admit Card page, where you need to enter your Registered Mobile Number and your Date of Birth and then click on the “Submit.”

Step 4– Now your call letter will appear on your screen. Cross check all the information printed on the call letter like your name, age, date of birth, address, your date, time and center of examination, etc., before blindly downloading the call letter.

Step 5– After thorough checking, download a soft copy of the admit card and then take a print out of it.

Documents Required Along With The Admit Card:

Along with a hard copy of the admit card, the candidates are also required to bring any one of the following government identity proof in original to verify the information and the identity mentioned in the admit card.

  1. PAN Card.
  2. Aadhar Card
  3. Passport
  4. Driving License
  5. Bank Passbook With A Photograph Attached to It.
  6. Voter’s Card
  7. College I.D.

Candidates are also asked to affix a passport size photograph the same one as uploaded at the time of online application on the admit card along with a photocopy of the government i.d. proof carried in original.

Cut-Off Marks Amd The Exam Result Date:

The department has fixed the cut-off marks for the online written examination to be conducted for the NHM CHO post. The cutoff marks stand as 33% for the Un-Reserved or General Category Candidates, 30% for the Other Backward Classes Category Candidates and 24% for SC/ST Category Candidates. But the exam result date and the merit result date has not yet been fixed by the department. They said that the result date would be announced post successful completion of the examination.
